Sign Up Process
There are three pages that need to be filed out: The Vendor Application & Contract , Vendor Information Form, and the Golf Sign-Up Sheet (if participating in the Charity Golf Tournament). After completion, please email to centralflfoagmail.com. If paying by credit card, please include the credit card number, exp. date, and the ccv#.
Booth specifications: 8x10 pipe and drape, 8’ table, 2 chairs, and company I. D. sign. Electric is an additional charge. Booth assignments are done on a first come basis.
